I/We being the registered owner(s) of the land described herein, am/are aware of the proposed outdoor patio, as indication on this application, and concur with the requested outdoor use.

Section E - Site Description & Information

Application is hereby made to operate on Municipal Property an outdoor patio fitting the following size (in m2) and capacity.

Section F - Alcohol

If you are serving alcohol on your patio, ALL regulations from the Alcohol and Gaming Commission of Ontario must be met in order to obtain your licence.

I hereby certify that 2 copies of a plan drawn to scale is attached showing:

• the lands to which the application applies (complete with detailed dimensions)
• the location of trees, light fixtures, hydrants, signs, etc.
• the general layout of permitted furniture and perimeter fencing
• all building entrances and dimensions

This agreement shall be renewed annually, prior to December 31 of each year. Outdoor patios are not permitted without a valid agreement. Appropriate actions may be taken by the Municipality where an outdoor sidewalk patio is established or continued to operate without the required agreement in place.

Section I - Freedom of Information

Please be advised all information collected will be used solely for the purposes of obtaining a patio licence, and will only be released through the Municipal Freedom of Information and Privacy of Protection Act, should it be necessary and approved by the regulations of the Act.
