I am the applicant for a permit for an Outdoor Patio.

In consideration of the granting of a permit for an Outdoor Patio by the Director of Planning and Development Services or Chief Building Official, I hereby release and forever discharge and agree to indemnify and save harmless The Corporation of the City of Thorold, its employees, elected officials, servants and agents from any and all actions, cause of actions, claims and demands for damages, loss, costs, expenses, or injury to persons or property, whether in law or in equity, howsoever caused, arising from or in any way connected to or sustained by any person or persons in consequence of the issuance of a permit for an Outdoor Patio being the subject of the permit application.

I understand that this release and indemnity applies to both present and future injuries and it is my intention that this release and indemnity shall bind my heirs, executors and administrators.

I HAVE READ THIS RELEASE AND INDEMNITY AND UNDERSTAND ALL OF ITS TERMS. I SIGN IT VOLUNTARILY AND WITH FULL KNOWLEDGE OF ITS SIGNIFICANCE.
